A workflow that looks clean in a diagram can still collapse in daily use. The most common problem is that teams document the happy path while leaving exceptions, ownership, and maintenance undefined.
Before launching any workflow, define who owns the trigger, who resolves stalled work, what data must be captured, and how the team will know whether the workflow is still helping.
The strongest launches include a rollback plan and a review date. That gives the team permission to improve the workflow without treating every adjustment like a failure.
